Registration

• If the student does not have a sports physical on file or if it expires before the end of a season, you will not be able to register.  Please scan and email a copy to TBrownlee@isd12.org.  It may take up to 24 hours to be updated in the system.
• Parent Portal is required to access activity registration. Student Portal does not have access. 
• Parent/guardian and students must agree to the terms presented, including the MSHSL Eligibility Brochure.
• Allow for about 10-15 minutes to complete required forms (Eligibility Brochure and Activity Participation).
• Related Centennial High School Athletic and Activity Fees are assigned and payment is required with registration.
• No practice or tryouts are allowed until fee is paid and Registration is completed, including having a current Physical on file.

1. Login to your parent portal account online or through the Parent Portal App on your mobile device.
2. Select Activity Registration
3. Select New Registration
4. Select Shop (High School Students will choose CHS Shop, Middle School Student will choose CMS Shop)
5. Select Products (CHS Athletics or CHS Activities)
6. Select View on the Activity to Register *If you do not see the activity available to register, the student does not have a sports physical on file or it expires prior to the end of the activity season. You will need to update the sports physical before you can continue with your registration process.
7. Select the Recipient to register
8. Select the link to complete the required 2025-2026 MSHSL Activity Eligibility Brochure
   *The MSHSL Brochure must be completed to continue the registration process.
9. Once the MSHSL Form is complete, select Add to Cart
10. Select My Cart
11. Add or Select Existing Payment Method
12. Select Submit Payment
    A receipt will be emailed to the email address linked to the parent portal account.**

In order to participate in a scheduled co-curricular contest/event &/or team tryouts, the participant is to attend all scheduled classes on that day. The student may not be truant any period. Truancy is defined as missing class or arriving to class more than 10 minutes late for unexcused reasons or leaving early without permission. In order to practice, the participant must have checked in and be in attendance by 11 am. Any exceptions to this policy must be approved ahead of time by the high school administration.

Activity participants must maintain a "C" average each semester to remain academically eligible. Students must be making satisfactory progress in accumulating credits for high school graduation.

1. Making academic progress toward graduation.

2. Will not have turned 20 before the start of the season in which I participate.

3. Have not dropped out of school or repeated a grade beginning with the initial entrance in the 7th grade.

4. Have not and will not use or possess tobacco or alcoholic beverages, use, consume, have in possession, buy, sell or give away any other controlled substance, including steroids, drug paraphernalia or products containing or used to deliver nicotine, tobacco products and other chemicals.

5. Have not and will not violate the racial/religious/sexual harassment/violence/and hazing bylaws of the MSHSL.

6. I agree to fully cooperate in any investigation honestly and truthfully.

7. Regardless of my age I agree to follow all of the MSHSL Bylaws in order to be eligible to represent my school in League sponsored activities.

8. Both the student and parent have reviewed the concussion management information contained in the Eligibility Brochure and found on the CDC website.

1. Students and parents must read and sign the Fee Packet.

2. Students must have a current physical examination form on file in the Activities Office. Physicals are only good for three years.

3. Insurance - Students must have insurance coverage under their parents or purchase it through the Student Insurance Program.

4. Students must maintain a cumulative GPA of 2.0 or higher.

5. Activity fee must be paid. 

6. The MSHSL data privacy guidelines are as follows: "Public information shall include names and pictures of students participating in or attending extra-curricular activities, school events, and High School League activities or events."

A transfer student is a student who discontinues enrollment and attendance in any high school, public or non-public, and enrolls in any other high school in Minnesota, or outside of Minnesota.  Essentially, a transfer occurs anytime a student’s school of record changes. A transfer is considered complete when the student attends class or participates with an athletic program at the new school. This includes home schools, charter schools, and online schools.
