Registration for CHS athletics and activities take place in your Parent Portal account. Registration for Spring Athletics is open Monday, February 3rd. Below you'll find links to register information, sports physicals and fees.

CHS Registration for Spring 2025 athletics
will open on Monday, February 3rd in Campus Parent Portal!

· MUST have a valid physical on file to register.
Sports Physicals can be emailed to TBrownlee@isd12.org
Once the valid sports physical is updated in your students chart,
you will be able to register in Campus Parent Portal.

· Visit our website for instructions for registration and/or how to view your student’s physical date.

CMS STUDENTS registering for CHS Athletics:
· If you had a sports physical for a middle school sport, you will
still need to send a copy toTBrownlee@isd12.org to update your students records.

Must have online registration forms and payment completed to begin participation
(This includes try-outs).

Registration Information

Important Information to Know Before Registering

If the student does not have a sports physical on file or if it expires before the end of a season, you will not be able to register. Please scan and email a copy to TBrownlee@isd12.org. It may take up to 24 hours to be updated in the system.
Parent Portal is required to access activity registration. Student Portal does not have access.
Parent/guardian and students must agree to the terms presented, including the MSHSL Eligibility Brochure.
Allow for about 10-15 minutes to complete required forms (Eligibility Brochure and Activity Participation).
Related Centennial High School Athletic and Activity Fees are assigned and payment is required with registration.
No practice or tryouts are allowed until fee is paid and Registration is completed, including having a current Physical on file.

How to Register

Login to your parent portal account online or through the Parent Portal App on your mobile device.
Select Activity Registration
Select New Registration
Select Shop (High School Students will choose CHS Shop, Middle School Student will choose CMS Shop)
Select Products (CHS Athletics or CHS Activities)
Select View on the Activity to Register *If you do not see the activity available to register, the student does not have a sports physical on file or it expires prior to the end of the activity season. You will need to update the sports physical before you can continue with your registration process.
Select the Recipient to register
Select the link to complete the required 2024-2025 MSHSL Activity Eligibility Brochure
*The MSHSL Brochure must be completed to continue the registration process.
Once the MSHSL Form is complete, select Add to Cart

Select Submit Payment
A receipt will be emailed to the email address linked to the parent portal account.**

Participation Requirements

Activity Attendance

In order to participate in a scheduled co-curricular contest/event &/or team tryouts, the participant is to attend all scheduled classes on that day. The student may not be truant any period. Truancy is defined as missing class or arriving to class more than 10 minutes late for unexcused reasons or leaving early without permission. In order to practice, the participant must have checked in and be in attendance by 11 am. Any exceptions to this policy must be approved ahead of time by the high school administration.

Activity participants must maintain a "C" average each semester to remain academically eligible. Students must be making satisfactory progress in accumulating credits for high school graduation.

MSHSL Eligibility

Making academic progress toward graduation.
Will not have turned 20 before the start of the season in which I participate.
Have not dropped out of school or repeated a grade beginning with the initial entrance in the 7th grade.
Have not and will not use or possess tobacco or alcoholic beverages, use, consume, have in possession, buy, sell or give away any other controlled substance, including steroids, drug paraphernalia or products containing or used to deliver nicotine, tobacco products and other chemicals.
Have not and will not violate the racial/religious/sexual harassment/violence/and hazing bylaws of the MSHSL.
I agree to fully cooperate in any investigation honestly and truthfully.
Regardless of my age I agree to follow all of the MSHSL Bylaws in order to be eligible to represent my school in League sponsored activities.
Both the student and parent have reviewed the concussion management information contained in the Eligibility Brochure and found on the CDC website.

Student Eligibility

Students and parents must read and sign the Fee Packet.
Students must have a current physical examination form on file in the Activities Office. Physicals are only good for three years.
Insurance - Students must have insurance coverage under their parents or purchase it through the Student Insurance Program.
Students must maintain a cumulative GPA of 2.0 or higher.
Activity fee must be paid.
The MSHSL data privacy guidelines are as follows: "Public information shall include names and pictures of students participating in or attending extra-curricular activities, school events, and High School League activities or events."

Transfer Student Eligibility

A transfer student is a student who discontinues enrollment and attendance in any high school, public or non-public, and enrolls in any other high school in Minnesota, or outside of Minnesota. Essentially, a transfer occurs anytime a student’s school of record changes. A transfer is considered complete when the student attends class or participates with an athletic program at the new school. This includes home schools, charter schools, and online schools.

Sports Physicals

If you are registering for a sport, a current physical must be on file with the Centennial High School Activities Office or you will be unable to proceed with registration. Physicals are considered current for 3 years and must not expire during the season for which the registration is being made.

Completed physical forms may be submitted to the Athletics & Activities Department at Centennial High School by emailing TBrownlee@isd12.org or faxing 763-392-7274.

I submitted my sports physical form to the office. When will I be able to register?

We typically ask for a grace period of 24 hours between the time the physical form was submitted to the time you can log on and register. The office staff is extremely busy and at times may not be able to process data or input information into the system right away. If you are still unable to complete registration after waiting the 24-hour period, please contact the office at TBrownlee@isd12.org. Please do not wait until the registration deadline to submit physical forms and expect to register that day.

Where do I turn in my Sports Physical Form?

Where should I take my child to get a sports physical examination?

The most common place to get a sports physical exam is the child's primary care physician or pediatrician's office. Any certified medical health professional can complete the physical exam and must sign and date the form.

There are select Walgreens stores and Minute Clinics that offer free sports physicals. You can search their websites to learn more information. Sport clinics like Summit Orthopedics also offer free sports physicals during the summer months and throughout the year. Pre-registration on their website is required.
